Secondary bone graft in cleft lip and palate patients

In recent years secondary bone graft in maxillary arch for cleft palate patients has become a reliable treatment .There are different surgical techniques such as segmental osteotomies , distraction osteogenesis and …Management of bilateral cleft and lip and palate is one of the most challenging procedures.
Secondary alveolar bone grafting is undertaken to achieve the following goals:
1)To form a continuous maxillary arch and create a satisfactory alveolar bony contour
2)To provide sufficient bone for eruption of teeth
3)To stabilize the premaxilla and palate in bilateral cleft patients
4)To prevent maxillary arch collapse
5)To provide bony support for the lip and alar base
6)To eliminate oronasal fistula
7)To improve facial esthetics and symmetry
8)To support dental arch width
9)To improve speech, mastication and oral hygiene
10)To allow ideal implant placement and create a natural soft tissue profile
Secondary bone grafting is most successful in mixed dentition stage prior to eruption of lateral incisors and canines.By postponing bone graft placement to completion of permanent dentition achieving the above mentioned goals will be rather impossible.
The treatment protocol was carried out on several nonsyndromic unilateral and bilateral cleft lip and palate patients mostly in mixed dentition. The patients had secondary bone grafting by an oral and maxillofacial surgeon and had orthodontic treatment pre and post surgery by an orthodontist.
A multidisciplinary treatment protocol for cleft lip and palate patients is presented.
